How do I make payment for each session?

To ensure that you understand your specific insurance requirements, we recommend that you contact your insurance carrier to determine your coverage, copayments/co-insurance, and overall plan.

What type of clothing should I wear to appointments?

Wear loose-fitting clothes that are easy to move in and allow easy access to the area causing your symptoms. We also recommend that you wear comfortable shoes or sneakers.

What should I do to get ready for my first appointment?

We ask all patients to bring a few items with them on their first visit to our office.

  • Please fill out the forms sent to you. If you cannot do so, please allow an additional 15 minutes time when you arrive to complete these forms.
  • Please dress comfortably so you can move around and so your therapist can access your problem area.
  • If you plan to use insurance, please bring it with you so we can make a copy for our records.
  • Please bring any referrals, prescriptions, medication list or results/notes from your medical provider.
  • Bring the contact names, phone numbers, and claim numbers of any case manager or insurance adjuster in the case of an automobile accident or worker’s compensation claim.

When you call our office, please have your prescription and insurance card ready. This will allow us to confirm your insurance benefits and provide you with a coverage estimate.

What are the office policies I should be aware of?

If you are unable to attend your appointment, please notify us at least 24 hours before your scheduled appointment time.  Any appointments that are missed and not rescheduled at least 24 hours in advance may be subject to a $70 charge.  We also ask that if you have been exposed to COVID-19 or are experiencing and COVID-19 symptoms please reschedule your appointment so that we can keep our patients and employees safe and healthy.

What should I anticipate from my first massage therapy session?

A health history questionnaire may be requested by your massage therapist. Following that, your therapist will ask you a series of broad questions to determine which areas require attention and whether any conditions require treatment. This will help them decide whether or not massage is right for you.

How long is each appointment going to be?
